WHAT IS SUPPORT COORDINATION?

Support Coordination is a service that helps you set up your NDIS Plan, find the right supports, and navigate the NDIS. To use Support Coordination, you must have specific funding for it in your NDIS Plan.

 

Melba’s Support Coordination will support you to source providers and services that meet your specific needs.

 

Our Support Coordinators will work with you and your network or help you develop a strong network of support, so you can achieve your NDIS Plan goals.

Support Coordinators give you the information you need to make informed choices based on their industry knowledge and expertise, so you can choose the right supports for you at any stage - even when life changes.

 

They are also able to work with you to build your own knowledge and confidence over time to coordinate your own supports, if you’d like to do that in the future.

 

Support Coordination can help you to:

  • Understand your NDIS Plan.

  • Create and understand your budget for your NDIS Plan.

  • Connect you to the right supports and services so you can achieve your goals.

  • Help to establish supports and services.

  • Build your capacity and resilience.

  • Understand service agreements.

  • Get your supports up and running.

  • Help you with any NDIS issues.

  • Report back to the National Disability Insurance Agency.

  • Help you to prepare for your NDIS Plan review.
