What is Individualised Support?

Our Individualised Support Arrangements (ISA) provide you with the support you require to access your community to focus on the things you want or to receive the support you require at home, or a combination of both. We provide you with one-to-one supports so that you can achieve your chosen outcomes in these areas.

Our services are flexible and tailored to your individual needs. We can support you with:

​

  • Assistance to maintain and develop skills to become more independent. For example, shopping, budgeting, cooking, household tasks, paying bills, using public transport, or problem solving

  • Support to pursue education and employment opportunities

  • Support with personal care needs, for example bathing or dressing

  • Support to go out in the community, for example going to the local gym, banks, shops or going to the footy

  • Assistance to maintain and develop friendships and social networks in the local community

  • Assistance to go to appointments with doctors, dentists, or community health services

Individualised Support is available in the following regions:

- Central Highlands and Wimmera (including Ballarat)

- Southern Melbourne (including Bayside Peninsula)

- Eastern and Outer Eastern Melbourne (including the Yarra Valley)
