Community Connections connect people with people. With three service sites based in Lilydale and Mt Evelyn, we offer programs and activities that are value-based, with the focus being on you and the support you need to achieve your NDIS goals. We offer centre-based programs or 1:1 support arrangements.

 

Our approach is based around you and your needs. We can assist you to develop a range of skills, support you to make the right choices in life and have control over decision making.
